「耳鼻咽喉科高度医療の砦を守る良医を育てる」

 金沢医科大学耳鼻咽喉科学教室は、1972年の本学開学とともに誕生し、2022年に50周年を迎えました。「内視鏡診療」の初代山下公一教授、「ナビゲーションガイド下手術」の第二代友田幸一教授、「嗅覚障害診療」の第三代三輪高喜教授へと渡ったバトンは、2025年3月に第四代教授として志賀英明に受け継がれました。私は金沢大学の学生時代に「聴覚生理学」に興味を覚え、僭越ながら当時の耳鼻咽喉科教授の人柄も師事するに足ると感じ耳鼻咽喉科の門を叩きました。入局後は米国留学を機会に一時期がん化学療法を専門としましたが、2006年から当時の三輪金沢大学准教授に師事して嗅覚研究の世界に飛び込み、2009年に本学へ三輪教授と共に奉職後も嗅覚研究を継続して現在に至ります。嗅覚障害診療に取り組む中で、最も多くの手術経験を積ませていただいたのが、慢性鼻副鼻腔炎に対する内視鏡下鼻副鼻腔手術でした。本学は三代の教授によって育まれた内視鏡、ナビゲーションシステム、そして嗅覚検査に恵まれた環境のもと、安全で機能改善を目指した内視鏡下鼻副鼻腔手術を行う体制が十分整っております。2020年に県内で初めて鼻科手術指導医制度認可施設に認定され次世代の鼻科手術執刀医を育成しております。さらに手術療法や標準的な薬物療法のみでは改善が難しい嗅覚障害に対する新たな治療法への萌芽も生まれています。以上の鼻科学の伝統の他、「味覚障害」、「睡眠時無呼吸症」、「小児難聴」、「めまい」、「中耳手術」などの専門家も揃っております。2013年に頭頸部外科学開講後は当教室で頭頸部がんを治療する機会はありませんが、外来診療では腫瘍性病変を的確に診断出来る若手医師の育成を頭頸部外科学と共同で行っております。

 当教室は特に能登地方の医療機関から多くの症例を紹介いただいております。また鼻副鼻腔疾患や上気道の急性炎症性疾患は県内全域から紹介を受ける他、嗅覚障害に関しては全国からも紹介を受けております。今後は高齢者の難聴などの検査治療も安心して任せていただける施設であることをアピールしていきます。教室員全員が地域の高度医療を守り抜くという使命感のもと、日々の診療に取り組んでおります。そして目の前の患者さんの病苦を除くために、知識と技術を磨き、地域で信頼される良医を目指しています。私共の教室から巣立った同門の先生方は皆、地域で大勢の患者さんに信頼されるクリニックの院長や病院の診療科長として活躍しております。私自身は、一生の仕事として耳鼻咽喉科医師が天職と思っております。今後、苦楽を共にする後輩の皆様に、自身が感じた耳鼻咽喉科の世界に出会えた喜びを伝えていきたいと思います。そして各人の適性に合った専門分野へ出会えるよう共に歩んでまいります。全国そして世界の医学生、研修医の皆様を石川県内灘町でお待ちしております。

Greetings from the Chair of Department,

"Training Good Doctors to Protect the Fortress of Advanced Medical Care of Otorhinolaryngoloy”

 The Department of Otorhinolaryngology of Kanazawa Medical University was established with the opening of the Kanazawa Medical University in 1972, and celebrated its 50th anniversary in 2022.The baton was passed on to Prof. Koichi Yamashita (The first Chair) for "endoscopic treatment," Prof. Koichi Tomoda (The second Chair) for "navigation-guided surgery," and Prof. Takaki Miwa (The third Chair) for "olfactory dysfunction treatment," and in March 2025, the fourth Chair, Prof. Hideaki Shiga, took over.

 When I was a student at School of Medicine, Kanazawa University, I became interested in "Auditory Physiology" and, without any presumption, I entered the Department of Otorhinolaryngology because I felt that the personality of the then Professor and Chair of Otorhinolaryngology, Kanazawa University was also worthy of studying under him.After joining the department, I studied in the U.S. and specialized in cancer chemotherapy for a while, but in 2006, I jumped into the world of olfactory research under then Associate Professor Miwa of Kanazawa University, and I have continued my olfactory research since 2009, when I joined Kanazawa Medical University with Prof. Miwa.

 In the course of my work on olfactory dysfunction treatment, I gained the most experience in endoscopic sinus surgery for chronic rhinosinusitis. Our university is well equipped to perform safe and functional endoscopic sinus surgery, thanks to the endoscopes, navigation systems, and olfaction tests developed by three generations of Chairs.In 2020, we were the first facility in the Ishikawa prefecture to be approved by the Rhinology Surgery Supervisor System. We are training the next generation of rhinologic surgeons. In addition, we are also developing new treatment methods for olfactory dysfunction that are difficult to treat with surgery or standard drug therapy alone.In addition to the above tradition of rhinology, we also have specialists in "taste disorder," "sleep apnea," "pediatric hearing loss," "vertigo," and "middle ear surgery," etc. Although we have not had the opportunity to treat head and neck cancer since the Department of Head and Neck Surgery has started in 2013, we are training young doctors who can accurately diagnose neoplastic lesions in the outpatient clinic.

 Our department receives many referrals, especially from medical institutions in the Noto region. We also receive referrals from all over the Ishikawa prefecture for nasal sinus disease and acute inflammatory diseases of the upper respiratory tract, as well as from all over Japan for olfactory disorders.In the future, we will continue to promote the fact that we are a facility that can be trusted for the examination and treatment of hearing loss in elderly patients.All members of the department are committed to the daily practice of medicine with a sense of mission to protect and preserve the advanced medical care in the community.We are also striving to become a good doctor trusted in the community by improving our knowledge and skills to eliminate the suffering of the patients in front of us. All of the doctors who have graduated from our department are active in the community as the director of a clinic or the head of a department in a hospital, where they are trusted by many patients.I personally believe that being an otolaryngologist is my calling for the rest of my life.I would like to share with my younger colleagues with whom I will share many hardships and joys in the world of Otolaryngology. We will work with you to help you find a specialty that fits your personals.
